Jayla Alexis Ward, 9 months, died Sunday, May 8, 2016 at Martin General Hospital.

Funeral services will be held on Thursday, May 12, 2016 at 2:00 PM at Community Christian Church with Rev. Dale Minton officiating. Burial will follow in Woodlawn Cemetery.

The family will receive friends Wednesday evening at the funeral home from 7:00 PM until 8:30 PM and other times at 13726 NC 125, Oak City, NC.
